The attorney must submit the following, by hand or mail, to "Clerk of Court"

Proposed Order for Attorney Name Change
(use the name you were admitted to practice under).
Copy of State Letter Certificate changing attorney’s name
Filing the Appropriate Fee for Name Change

Request Attorney Info

If you need to request an attorney’s date of admission, bar number, log-in or password you must e-mail your request to:

helpdesk@nysd.uscourts.gov

Your email request must include

The attorney’s full name
The firm name
The last four digits of the social security number
The state bar number
The attorney’s e-mail address

If the request is received during business hours, you should receive a response the same day.
